THIS IS A ONE-TIME PROCEDURE! Get your Plum Market Rewards Card at your favorite Plum Market Guest Relations desk.  If you already have one, the Plum Market Rewards Card needs to be connected to FJA by doing one of the following:

Call Rewards Member Care (248-487-7007) and indicate you would like to participate in the scrip program.  You will need to provide:
        First and Last Name
        Reward Card number (on back of card, under bar code)
        Your organization

OR:

Fill out a Scrip Enrollment New or Transfer form at Guest Relations and provide the information above.

It will take 3-5 business days for processing.  Swipe your Rewards Card each time you check out so that FJA receives the 5% donation.
